This Ontario College Graduate Certificate program is designed for graduates of a diploma or degree in nursing seeking knowledge and skills to work in settings that care for individuals, families, and communities focused on improving health and healthcare outcomes through health promotion, health education, disease prevention, and management of chronic health conditions.
Building on your healthcare knowledge, you will learn about foundational concepts and approaches in health promotion, social determinants of health, chronic disease prevention and management, and supporting patients with behaviour change for self-managing chronic diseases throughout the lifespan. This program culminates in a major capstone project applying the theoretical and practical knowledge you acquired into a creative and comprehensive summative.
